Service Requests

District maintenance and technology requests are submitted and managed through separate systems. Please select the appropriate option from the choices when submitting a request.

Maintenance/Facilities Requests

Use this form to submit to submit MAINTENANCE requests. These are typically issues with buildings and/or classrooms that involve:

  • Electrical

  • Heating, Ventilation, and Air Conditioning (HVAC) work

  • Lighting

  • Repairs

  • Requested Modifications

If you have a need for cleaning or cleaning supplies in your classroom, please submit a Custodial Feedback form (see below).

Custodial Feedback form

Use this form to submit feedback regarding the custodial work being done in your learning environment, as well as submit requests for cleaning type (custodial only) items.  If you have positive feedback you'd like to share, or constructive feedback or areas of improvement that need to be addressed, fill out this form.  Any requests or concerns will be communicated to the appropriate staff within 24hrs. If your request/feedback is URGENT, please see your building principal and/or custodian.     ***This does not replace the Maintenance Request Form***

Information Technology (IT) Helpdesk

These are typically issues with:

  • Computer Hardware/Software

  • Internet Sites

  • Phones

  • Technological Devices

  • Wired and/or Wireless Access

There are two methods to submit IT helpdesk tickets:

  • Send an email to This will open a ticket in or system. Please be detailed.
  • Visit, login using your district Google Account and click on "Report an Issue". This is the perferred method as it allows for more information to be populated.
